Case History Report

Student background/Parent interview:

Jason is a vibrant five-year-old boy who is the only child in Kent’s family. He has blonde hair and his blue eyes show subtle intelligence and shyness. Being the only child, he is greatly adored by his mother and father. He is currently studying at Burton Valley Elementary School. According to his mother, he shows much promise in his studies and she is fairly certain that he is on the right path towards a healthy career. The interview was a lengthy discussion with his mother about areas concerning his academics, behavior, and social-emotional functioning.

The mother was eager to tell about Jason’s development from infancy to his current age. She said that Jason had experienced a normal childhood. Mrs. Kent explained that Jason had accomplished the relevant required milestones for a six-year-old. His mom adds that he has no problems associating with other kids. She explains that he actively participates in school activities and clubs. The mother added that he spends most of his time playing video games and solving puzzles with his father. According to the interview, the mother expressed a sense of hope in the boy’s future.

Observation #1

Jason was observed in his classroom on the 25th of June at 10:00 am. He currently attends Burton Valley Elementary School. His classroom is attended by nineteen other students and a teacher who supervises how they learn. The topic of the day is reciting the alphabet. Jason is seated in the second row adjacent to a girl nearly his age. Judging from his posture, he seems to be very attentive to what the teacher says. Jason and the class are using notebooks on their desks to sing along with the teacher who is actively singing the alphabet song. He is the most fluent with his alphabet in his class and he gets the most positive reviews from his teacher.

Jason uses the images in the notebook to understand the alphabet. He spends more time on the pages with relatively new images as if he is trying to understand what the pictures mean. He slightly stutters with the letters that are harder to pronounce. The teacher is eager to help him out by using more relevant examples. After, the period, he is the last one to place his notebook on the shelf. He is very calm and subtle with his actions. He continues to observe some charts on the classroom wall as he waltzes outside the classroom.

Observation #2

Jason was observed again on the 26th of June at 10:00 am in the school gym during his physical education lesson. He casually enters the gym room in his games attire. Other students and a gym teacher proceed to enter with him. The class was enthusiastic about this period judging from the smiles and energy that was in the air. The session began with a game of dodge ball. Two teams of ten players were selected and were aligned opposite to each other. Jason was among the first picked by the team leaders showing that they appreciate his abilities in the field. His enthusiasm for this sport was reflected by how he managed to miss almost all the balls thrown at him by the opponents.

After about twenty minutes of the game, the students are allowed to free play in the gymnasium. The gymnasium has equipment such as ball pits, swings, trampolines, ladders, trapeze bars, and many other exercise tools. Jason is particularly interested to be with his friends. He follows them and does whatever they are doing. He is happy to be with them because he smiles and laughs a lot when he is around them. They play with the trampoline for a while before the period is over. As soon as they all leave the gym, the observation was concluded.

Teacher interview:

The observer then spoke to Jason’s teacher. The interview took place in Jason’s class while no one else was there. The teacher revealed to the observer that she was especially impressed by Jason’s academic accomplishments. Her face lit up as she talked about his class attendance and his overall participation in in-class activities. She explained how Jason was among the first students in the class to memorize the first ten letters of the alphabets. Socially, the teacher described Jason as quite shy around people he does not recognize. She added that Jason was very talkative with his friends.

The examiner then asked her whether Jason was frequently found with indiscipline activities. The teacher confirmed that in school Jason is among the most well behaved. She said that he is a smart, generous, and caring boy. The teacher was positive that Jason was a role model for other students in the class. The interviewer then asked whether Jason appears to have emotional problems. The adamant responded by explaining how he has a healthy adamant reaction to his surroundings. She says that he is sad when he feels pain and happy when he feels good. She is adamant about Jason’s bright future.
