Theory research and practice
Evidence-based research is very crucial in day to day decision making in various fields. Evidence-based research refers to basing information used in decision making inpatient care on research rather than opinion. It means that a number of sources from previous studies that have been published in medical journals must be reviewed for results, data, and conclusion of reputable, valid studies. The end goal is to make better clinically made decisions and to offer exemplary care for patients. In nursing, evidence-based nursing involves intense methodology whereby summative data of research for certain problems are analyzed in conjunction with patient’s preferences and values and professional skills and expertise.
The significance of theory in evidence-based research is based on the insinuation that scientific evidence is insufficient and that predictive theory capability and recourse to explanations are important to the design of evaluations and programs. The use of theory is basically for the enhancement of comprehension of convoluted situations. There are two relevant types of theories that promote the planning cycle in health. They include the change theory and the explanatory theory. Change theory guides implementation development of interventions means while the explanatory theory gives direction in the nature of the challenge and aids in the identification of factors that ought to be modified by a health promoter. Lack of rational assessment of the problem and providence of possible solutions, interventions are likely to address inappropriate variables or handle sects of variables that bare likely to give certain results. The theory also lays a framework for verifying whether there is the availability of the required elements of a program.
Research findings are an integral part of evidence-based research. They improve on the decisions made to benefit patients. Research findings help in developing strategies for changes in organizations. The whole idea of theory evidence and research finding has its centrality in aiding to make informed decisions to better the outcome of patient treatment and ensuring success in the providence of health services. The practice in evidence-based research is very utilitarian in that it creates a ground for substantive aid to patients as well as making health care plan a success.
The article I have chosen is ‘the efficacy and safety of an RN-driven Ketamine protocol for adjunctive analgesia during burn wound care’ the article revolves around how analgesic regimens fail to manage excruciating pain that is experienced by patients when they have a burn wound care, and how the frequently administered drugs would lead to respiratory depression and sedation (Smith, 2016). The article further reveals how ketamine would be a good option for analgesic patients due to its limited action mechanisms and the deprivation of link with respiratory depression. The study was geared towards an evaluation of safety and efficacy significance care RN driven protocol of administration of IV ketamine administration in case of a burn wound-care. The cohort study that was retrospective examined patients administered with ketamine as part of a vital care RN guided by the burn wound care by ketamine protocol. Through evidence-based research, it was possible to arrive at the findings that high doses of anxiolytics and opioids require pain control when a burn would care is being treated but also they provide for a chance of adverse long term and short term effects. The short term effects later lead to long term effects, which translate to tolerance to medicine and thus requiring high doses of medicine that aid in pain control. The use of a theory-based ideology is what trickles down to the outcome of the research. Although there is no empirical knowledge of the administration of ketamine to patients, the study has been propelled by the aspect of a theory evidenced by research. The presence of a theory helps to decipher complex aspects of a study and thus providing different perspectives of carrying out research. It also acts as a caution to the extent to which a researcher can go and the route he should follow. Moreover, a theory is able to help in projecting the results that are bound to show at the end of a study. Additionally, theory enhances the creation of contingency plans just in case of challenges in a study. In the article on the safety and efficacy of the usage of ketamine protocol for wound care, the patient who received the medicine had a great preference for it. Through this finding from the research, it was possible to offer patients friendly care. The realization that patients who were offered ketamine orally greatly improved their pain score than those administered with dexmedetomidine helped clinicians to choose the right treatment for patients.
It is imperative to clearly identify a conceptual context so that specifications of a model can be used to determine what part of the theory explains, describe, and predict the outcome of a research. Identification of a model places a study in an intended socio-historical and intellectual context. Therefore, every conceptual approach provides a basis that controls the queries that one might have and the theories likely to be proposed and tested. The best conceptual model for this particular research is Levine’s conservation model. Its assumptions guide this particular model’s choice that nurses provide an environment in which the patient is bound to get proper treatment and that people are likely to have their healing process predicted (Fawcett, 2016). Also, the model relies on the fact that human beings are more likely to be unique in the way they respond to treatment. The four principles of the conversation model guided how the growth of the intervention exercises and the recognition of the salient results for those under treatment and the choice of the right instruments to measure variables under study. The reason as to why this model would have been important for this model is that it would facilitate the testing of model-based interventions and the design together with the development of empirical backup to support the practice of nursing.
In conclusion, it is important to note that a theoretical framework or what is also known as a conceptual model provides an orderly and unified means of comprehending events that are related to relevant processes of a discipline. Through this kind of understanding and involvement of evidence-based research, the results of the research are bound to be objective.
