TASK INVENTORY WORKSHEET INSTRUCTIONS:
Step 1: The job for this assignment is Alligator fishing. Next you are to identify some (at least 10) of the tasks involved in performing the duties (both routine and non-routine tasks) based on the YouTube video.
Step 2. After identifying the general tasks identify the general loss exposures (column 2) expected. Harm or property damage which can occur during each step of the job. Hint: Remember from previous risk evaluation you have done a similar assignment previously.
Step 3. Next perform a risk evaluation. Use the following formula.
Severity of Risk Exposure: Use a scale of 0 – 6.
0: equates to No injury or Illness or quality, production or other loss of less than $100.
2: Minor injury or illness without lost time; non-disruptive property damage; quality, production, environmental or other loss $100 – $1000.
4: A loss time injury or illness without permanent disability; disruptive property damage; quality, production, environmental of loss more than $1000 but not more than $5000.
6: Permanent disability; loss of life or body part; extensive loss of structure, equipment, material; quality, production, environmental or other losses exceeding $5000
Frequency of Exposure: Use a scale of 1 -3. Few: Less than daily (1); Few times a day (1) Many times a day (2)
Moderate: Less than daily (1); Few times a day (2); Many times per day (3)
Many: Less than daily (2); Few times a day (3); Many times per day (3)
Probability of Exposure: Use a scale -1 to +1: -1= Low probability of loss
0= Moderate probability of loss
+1= High probability of loss
Critical Rating: Total the severity, frequency, probability and this becomes your critical rating.
Check all the elements (program needs) also considered recommendations that will address the tasks, loss exposures, and risk for the items which have the highest potential of risk for harm or property damage.
