Brothers is a film that was produced in 2009, which mainly entails psychological drama. It involves Sam Cahill, who is a marine captain and starts with him being about to be deployed on his fourth combat. He is a family man as he is married to his wife Grace, and they raise two daughters together. At the same time, Tommy, Sam’s brother, is being released from prison after being convicted for an armed robbery. Post-traumatic stress disorder (PTSD) is a mental health disorder that is mainly caused by an event (Kessler et al. 2017). A person experiencing the condition mainly has flashbacks, severe anxiety ad also nightmares. Sam, in the movie after going to the combat, comes back home and starts experiencing PTSD.
The combat is in Afghanistan, and he returns to the combat, but on the way there, their helicopter crashes, and even though he was thought dead, he survives the crash. He is, however, being held captive by the Taliban, a group of Muslims. Tommy, on the other hand, while at home, he man’s up and takes responsibility as he comforts Sam’s wife and children during the time that he is pronounced missing. Sam, together with Joe, his friend are tortured by their captors during which they are forced to make dismissals about their mission. Only Joe opens up, and therefore the captors deem him useless, and they force Sam at gunpoint to kill him.
He is then resumed, after which he goes home back to his family. Once he gets home, he shows signs of PTSD as he has a cold and a paranoid daze, which can be related to his captors’ traumatic experience with his captors in Afghanistan. He refuses to share with his family what happened ad even goes ahead and lies to Joe’s widow, saying that he does not know what happened to him. He becomes so paranoid that he starts believing that Grace and Tommy have a relationship.
During his daughter’s birthday party, Sam becomes enraged and holds a gun at Tommy, but he calms him down, after which he holds the gun to his head. He surrenders the gun after Tommy and Grace Plea with him, after which the police arrest him. He is then admitted to a mental hospital where Grace visits him and forces him to explain what was happening. He finally opens up, and they reconcile. As they hug, we hear Sam through a narration, wondering whether he will live a normal life.
Taking Sam to a mental hospital was the right treatment plan since he had already proved that he could bring harm to himself and others. It was clear that he had PTSD mainly because he had killed his friend Joe. He tried to kill Tommy since he was suspicious of him and his wife and then pointed the gun at himself, trying to commit suicide. He was also violent when he confronted his wife about cheating on him as he started breaking everything in the kitchen, after which he held Tommy and pointed a gun at him. Therefore, he needed help from a mental hospital facility since he could be well monitored when he was there and receive the care and attention that he needed.
There he could receive psychotherapy sessions to calm him down. Based on his unique skills in that he was a soldier, there was a need for him to be put away somewhere where he could be monitored to prevent him from harming others. Being that he is a soldier, he is from a good socio-economic status, and he is taken to a good mental hospital to receive treatment. Therefore, the treatment plan was customized to suit the patient’s unique skills, socio-economic status, culture, and lifestyle.
Some of the techniques that were used in order to facilitate effective communication is an ultimatum which was given by his wife. When Sam was admitted to the hospital, the wife did not quite fully understand the reason behind his violent actions where he tried to kill himself and Tommy, and therefore she asked an explanation from Sam. Earlier on, Sam can be seen to shut off her family members as he refused to tell them what had happened to him to behave the way he was. This time, Grace told Sam that if he did not tell her what was happening to him about his pain source, he would lose her forever. Since Sam did not want this to happen, she tells Grace everything, including how he killed Joe. Grace empathized with him and comforted him, which is also a communication skill used in the movie.
There is no clear therapeutic relationship shown in the movie, as we do not see Sam interacting with the hospitals’ caregivers. It is only shown when he comes out of the hospital, and he seems to be sound while he was talking with his wife. The patient’s strengths were incorporated into the treatment plan since Sam had the strength of interpersonal relationships as he was very close to his wife and brother, who supported him. He was allowed to see his wife, who he loved very much, and she was part of the reason why Sam seemed to be getting better.
She talked him down and convinced him to tell her what was happening, which is very important for patients with PTSD to do since they can be helped after opening up (Shalev et al. 2019). The patient did not participate in developing the plan since it was not until later when he confessed what was disturbing him to his wife. All along, he refused to tell his wife and the people close to him why he was acting up.
Therefore, Sam had PTSD, where he was suffering from the entire trauma that he had gone through in Afghanistan. He was forced to kill his friend Joe and had gone through a lot of torture there. When he returned, he thought the brother and his wife were having a relationship, which made him angrier. He starts acting up, trying to kill the brother and himself, and that is when he is taken to a mental hospital. The treatment plan was the most suitable one since he was becoming a danger to himself and others and therefore needed to be in a place where he could be closely monitored.
