In my community, cardiovascular disease is one of the community health problems. The disease has recently become the leading cause of death to the majority of the people in the community. Cardiovascular disease often referred to as heart disease is a health complication that refers to a condition where the blood vessels are blocked or narrowed. Also, the disease affects heart valves, muscles, and heartbeat rhythm leading to a stroke or heart attack. Strokes and heart attacks are the acute events of the disease caused by blockage which prevents the free flow of the blood to the brain and the heart. The development of this complication majorly relies on prevalence factors which include unhealthy diet, use of tobacco, alcohol, and lack of physical activities. The high death rate caused by the disease creates the need to develop a community health improvement plan to reduce the number of deaths.
Structure evaluation
To address the problem of cardiovascular disease in the community, an outstanding community health improvement structure must be developed. The structure of the program should entirely focus on community collaboration in addressing specific health issues and more so monitoring overall health matters. The structure should enhance the development of exceptional community goals aimed at improving the health of the people in the community. Additionally, the structure must constitute integrated ways to achieve the stated objective of the program.
Process evaluation
In evaluating the process of the program, the operation of the program must entirely rely on the action, analysis, and measurement of the problem in the community. In the process of identifying the problem caused by cardiovascular disease, the program has to monitor the health indicators at the community level and identify intervention measures to be undertaken. In the process of implementing the stated objectives, the program must assess the resources to be used, and selecting the stakeholders meant to implement the program.
Also, in evaluating the process of the program, it is crucial to determine whether the action undertaken by the program conforms to the health determinants and its broad view. The program must provide an exceptional conceptual basis that impacts the health of the people in the community. The program process must be in a position to use evidence-based approaches in determining the intervention measures that best suit the health issue to be addressed in the community. Pieces of evidence are vital in accurately determining the health factors which contribute to the advancement of the health complication in the community and also when choosing the appropriate intervention measure.
Outcome evaluation
In evaluating the outcome of the community health improvement plan, it is important to consider the evolving and iterative nature of the program and also the ability of the plan to solve the health complication in the community. The plan must be seen to be evolving or iterative rather than being a short term or linear. The outcome of the plan must not, in any case whatsoever, hinder efficient and effective operations meant to address specific health issues in the community. Also, the community health improvement plan must accommodate the dynamic nature of communities and facilitate the effective flow of information among the accountable stakeholders.
