Prevention and Risk for Diabetes Type 2

 

Epidemiological Rationale for the topic:

As has been observed in recent years, many people suffer from this condition and end up dying. Physically active people do not suffer much from this condition. This makes it essential for people to observe diet and get screened, mainly if one is above 40. Home health care practice should also be carried out where all health care services are administered at home bases when one is suffering from this condition (Chatterjee et al., 2017).

Readiness for Learning:

The members will envelop their viewpoints concerning diabetes type 2 counteraction draws near and pose reasonable inquiries regarding reducing hazard factors, participating in the conversation, and verbalizing understanding. The members will also be taught the importance of observing homecare health services (Broom & Whittaker, 2004).

Goal evaluation plan:

I will follow up with all members in their homes to ensure that they have taken keen on measures that help reduce the risks of diabetes type 2.This will help reduce the increase of patients suffering from this disease.

Plan for the process evaluation:

I will give some questions during the training to ascertain if the members have been familiar with the topic. I will ask them if the training was well conducted and if it had achieved their expectations. I would also ask them to rate the practice to know what to improve next time.

Obstacles hindering practical training:

Some members may be uncooperative, while others may fail to attend the training. This will hinder the purpose of the training from reaching many people, and hence homecare health services may not be achieved fully. However, the solution for these barriers may be performed by advising the family members to observe lifestyles.

Communication:

I will start first by introducing myself to the audience and telling them about the topic. Then I will seek to know if they have any understanding of diabetes and its relationship to the homecare health services. I will strive to see if they know the causes, consequences, and solutions to diabetes to establish the starting point.it is at this point that I will let them discuss among themselves as they take their refreshments.
