Summary of intervention and implementation
What I did
I provided a presentation about ways of managing diabetes and preventing it from affecting the health of older adults. As such, the audience were patients, their families, and other healthcare givers. After the presentation, I provided handouts to all the three groups that contained more information and will act as a point of reference in the future.
How I led terms of the process used in the project
I explained the terms of the process used in the project. As a result, all key terms were vividly described to the patients and their families to ensure that they get all information being disseminated.
The participants
The participants include the patients, their families, and other healthcare professionals (who wanted to increase their level of knowledge on diabetes. Thus, they were engaged in continuous medical education (CMEs)).
Interprofessional collaborators
The interprofessional collaborators include physicians, nurses, social workers, clinical offers, and pharmacists who want to see the best care.
How I lead them in the project
I led them in the presentation. I explained how the number of diabetes has been increasing since the turn of the century. I told the steps that have been taken to prevent the problem and two primary interventions that can be useful in preventing the infection.
Key findings
The key findings were that the number of cases of diabetic patients has been on the rise since 2000. As such, prompt diagnosis and treatments and education are useful in preventing the condition from affecting the health of the elderly (Menke et al., 2015).

The professional product used is the handout. The handouts are given to the patient, their families, and other healthcare givers after knowing how diabetes could be prevented among older adults. As such, older adults, patients, and other healthcare givers will use the handouts as points of reference.

The use of patient handouts provides detailed information about the issues being discussed about diabetes and ways of preventing it from affecting the lives of older adults. As a result, the audience is given something to carry home after finishing the presentation or disseminating information. In this regard, they will be using the information in the literature to make informed decisions about their health. The use of handouts is one of the critical ways of reminding the patients, their families, and other healthcare givers about the key messages that were being disseminated. As such, they will be using the patient’s handouts as a point of reference. Because of the reasons surrounding diabetes are complex and technical, the patient’s handouts are useful in ensuring more information that the patients will use for guidance. The handouts can explain the key messages that were provided to the patients. The handouts will be serving as a summary of the main points. As such, detailed information can be available, which will enable the readers to make informed choices about their health.

The use of handouts helps improve the health of patients and quality of life. As such, they will be promoting adherence to prescribed behaviours. The primary interventions include the prompt diagnosis and treatment of diabetes and the use of education in managing the complications arising from the condition. As a result, handouts are an effective way of instilling these behaviours to patients, which improves knowledge, skills, and choices in managing the disease. Thus, the handouts are useful in promoting patient-centered care, which implies that they will be adhering to the treatment/medication and therapies geared toward managing the condition.
The importance of the product in improving outcomes
The primary reason for using the handouts is to ensure an improvement in patients’ health and quality of life. Handouts have been proven effective for patients, as they promote adherence to prescribed behaviours. Besides, there is also an increase in the level of knowledge for patients, skills, and values, which is useful for making informed choices. The diabetic condition is not only controlled by drugs but also diet and exercise therapies, which enable the patients to manage the situation effectively. As such, healthcare providers must find ways of disseminating information to patients. One such method involves using the patient’s handouts, which improves their knowledge/skills to better the healthcare outcomes.
How handouts enhance patients experience
The handouts are useful in educating the patients about ways through which they can use in managing the diabetic condition. They become informed about the importance of early diagnosis and prevention of the infection, which will help prevent complications. Besides, the patients and their families will get information on how to manage the disease through exercise, physical activity, changing dietary habits, and the use of medications. Because of the enhanced level of knowledge, the patient’s experience is improved. They will get satisfied with the care provided in the healthcare facilities. It is because of the improvement in the level of expertise.

The medical profession does not only involve the process of providing treatment to the patient; it also consists of the process of improving the level of knowledge for patients. The use of handouts ensures the promotion of patient-centered care enabling the patients to adhere to therapies, medications, and treatments. As a result, there will be increased compliance, which ensures the delivery of cost-effective healthcare delivery patients (Xu et al., 2018). The process of educating patient using handouts provide continuous care, which will be reducing complications caused by diabetes. As a result, there is an improvement in safety, quality, and healthcare costs when handouts have been used. The

The evidence provides that the delivery of patient-centered care involves improving the level of knowledge for patients. As such, patient education on the importance of prompt diagnosis and treatment of diabetes and patient education is critically important in delivering patient-centered care, which will be increasing adherence to medication and treatment plans. As such, the patient will be adhering to medication and therapies. Hence, handouts are an effective way of delivering more information on how to manage and prevent diabetes from affecting older adults’ lives.
