What I did

I provided a presentation about ways of managing diabetes and preventing it from affecting the health of older adults. As such, the audience were patients, their families, and other healthcare givers. After the presentation, I provided handouts to all the three groups that contained more information and will act as a point of reference in the future.

I explained the terms of the process used in the project. As a result, all key terms were vividly described to the patients and their families to ensure that they get all information being disseminated.

The participants include the patients, their families, and other healthcare professionals (who wanted to increase their level of knowledge on diabetes. Thus, they were engaged in continuous medical education (CMEs)).

How I lead them in the project

I led them in the presentation. I explained how the number of diabetes has been increasing since the turn of the century. I told the steps that have been taken to prevent the problem and two primary interventions that can be useful in preventing the infection.

The process of educating patient using handouts provide continuous care, which will be reducing complications caused by diabetes. As a result, there is an improvement in safety, quality, and healthcare costs when handouts have been used. The

The evidence provides that the delivery of patient-centered care involves improving the level of knowledge for patients. As such, the patient will be adhering to medication and therapies.
