The use of telemonitoring devices will be useful in monitoring patients’ health recuperating under the home-based care to ensure a reduced number of rehospitalization (Anttalainen et al., 2016). It has proven effective in monitoring the patient’s vitals, which provides data to the healthcare givers. They act immediately to ensure corrective action is taken when the health of patients is deteriorating.
Secondly, the electronic health record will be useful for patients with diabetes. It is a real-time patient-centered that is useful to provide information about the medical and health histories about patients with the condition. As such, the healthcare professionals will obtain diagnosis, medications, and treatment plans about the patients. They will be using this information in making decisions during the provision of care services. The primary importance of the HER is that there can be creation and management of the data by healthcare providers in a digital platform. The information can be shared between the healthcare facilities and nursing homes, which will help improve healthcare quality.

It also provides guidelines helpful in reducing the rate of infections by ensuring accessibility of healthcare services for patients to get prompt diagnosis and treatment of diabetes. As such, organizations that have an increased rate of rehospitalization have to pay penalties.
Healthy People 2020 is a useful policy in ensuring that people with diabetes attain a high-quality life. As such, they have an experience that is free from preventable conditions. There will be a reduced rate of mortality for people with diabetes, as the healthy people 2020 policy ensures that there is an elimination of the health disparities and improved quality of life for people. There will be an improvement in health for all people with DM.
The use of insurance policies is critically important for people with diabetes problems. The purpose of the Medicare and Medicaid system ensures coverage for people with health problems like diabetes. Medicare is a government health program that is useful in providing health for people above 65 years of under 65, regardless of the economic income. Besides, Medicaid is helpful in the provision of health for people who have a low income. Hence, it is useful in improving patients’ quality of care because they can pay their healthcare costs.
The last policy is the HIPAA policy ensures that the patient’s information is protected. They do not get into the hands of malicious individuals who may get access to the healthcare data. Cybersecurity has been one of the challenges affecting healthcare organizations. The use of the HIPAA act ensures the protection of patient’s data from cybercriminals.
Strategies for improving communication
The promotion of effective communication between healthcare givers, patients, and their families is an essential strategy in delivering care. There will be constant sharing of data between the healthcare providers and the patients and their families to air their concerns about the care process. The assertion of ethical values is useful in dealing with cultural barriers and differences that may affect healthcare delivery. Also, the need to involve the patients and their families in the care process ensures that there will be collaboration and effective communication.
