Lewin’s model will be used as the preferred change theory in implementing the proposed interdisciplinary plan. This change model is quite simple since it has only three steps for the implementation process to be complete. They are unfreezing, transitioning, and freezing (Burke, W. 2014). Unfreezing tries typically to reduce the need to maintain social status as well as altering the present attitude among people or institutions. In this scenario, the children will have to change their attitude towards their current eating habits. Unfreezing will achieve by trying to introduce some new eating healthy habits which children will have to adopt. The interdisciplinary team will ensure that the children realize the need for change. The transitioning step then follows whereby the children, in this case, will implement what they will have learned. They must develop those new behaviors after seeing what their old behavior is doing to their health. For instance, the children will leave their present lifestyle of watching TV all day after consuming so much processed foods. They will adapt the norm of going out to play with friends to break down the cholesterol. The last step of freezing crystallizes the new rule after the implementation has been completed (Burke, W. 2014). The organization or individuals could revert to their old ways of living unless they reinforce the made changes via freezing.

Leadership Strategy

Proper coordination is one of the leadership strategies that will assist in meeting the interdisciplinary team’s objectives. All health professionals have chosen to be part of the club organize themselves into groups that participate actively in the plan. Effective coordination by the team leaders ensures everything runs as expected, and everyone completes their assignments (Wiseman, B., & Kaprielian, V.S. 2014). The involvement of other health professionals in the decision making and planning process ensures coordination, and they acquire a sense of belonging. Proper coordination ensures timelines are met, and tasks are accomplished according to the leaders’ expectations. Finally, the engagement of employees by leaders in policy and decision-making processes enhances their strengths. Engagement empowers the health professionals in their tasks, thus improving their performances, promoting health among patients.

Collaboration needed to achieve set objectives.

Interprofessional collaboration involves all team members in a health facility to work together and ensure patients are treated effectively since there is sharing of information concerning different conditions, (Kos, M. 2019). All members are expected to interact with the patients as they share personal but useful information that could help in their treatment. Leaders are expected to drop their pride for the patients’ sake during interprofessional collaboration. Many facilities encourage team meetings and patient-centered and team-based rounds, including bed nurses, primary doctors, bedside nurses, physicians, and other relevant members (Mensik, J.S. 2013). Interprofessional collaboration fosters patient-centered care in healthcare and allows hospital technology to support teams to collaborate securely at the point of care through voice or text.

Additionally, interprofessional collaboration reduces medical errors, thus improving patient outcomes, minimizing healthcare costs. Medical facilities can save money by redundancies of the workflow as well as operational inefficiencies. In our case, team members from the Peterson Center will ensure that adequate preparations are made before implementing change. For example, all the resources required by the staff and children are made available before the scheduled time. Some can visit the schools meant to conduct the training one day before the d-day to ensure the hall and other necessities are in order (Mensik, J.S. 2013). They will also ensure their colleagues are prepared for the change as they will also be indirectly affected.

Financial and Organizational Resources

The organizational managers from the Peterson Center will make sure that the needed requirements are available. Physical resources such as projectors are required to use during the training of the children.

The management will ensure the intervention is implemented successfully. The information technology staff within the facility will ensure their colleagues can access medical records belonging to patients without delay. Thus, it will make sure that health care providers are competent and accurate.

The team members discussed the budget and presented it to the management. The most effective collaborative approach implementation of the change is the sharing of information. The administration should ensure every team member is aware of the plan at hand so that all operations can run as expected. Members of the Peterson Center interdisciplinary team will share information through one to one communication, mobile phones, and emails. Donations by the relevant stakeholders were also communicated during meetings held during the planning process (Broadbent, M., & Cullen, J. 2015). Cooperation will, therefore, be boosted when all team members work together to achieve the desired outcomes.
