The patient is the individuals who are waiting or are under medical care and treatment. They act as the recipient of the different services provided by the health practitioners where they are worked upon to enhance their recovery from various health complications (Alghamdi, 2016).
Consequently, family nursing entails the section of the primary care given to the patient of all ages, ranging from the infants to geriatric health (Dulai, Parambir, et al. “2018). The aid facilitates the strong relationship among the healthcare provider-patient and the family to uphold and provide better supportive care in healthcare.
The population health problem involves the health outcome of the group of individuals in the community. The result creates the problem that causes the physical condition to the individual, which is fostered by the abnormal functioning and unbalanced state of the body.
Realistic goal-relevant in personal practice is the best articulate tool that boosts the monitoring of the professional nurse progress and provides the foundation of the groundwork that enhances the career path advancement. Some of the realistic goals that the nurse should attain in the line of duties include, keeping time, to be specific, stay accurate measurable, keep the goal attainable, being realistic and saving time in the personal practices.
Hypertension is one of the most common health problems that most of the American population is suffering from. Notably, the complication develops when the blood flow via arteries is relatively higher than the normal pressure. The blood pressure is made of the diastolic and systolic pressure. The isolated systolic hypertension is the blood pressure that is above 120mm/Hg, while the diastolic blood pressure is always below 80mm/Hg. The normal blood pressure is supposed to be less than 120 and over 80 millimeters of mercury (Carey, & Whelton, 2018). Mostly the kidney act as the boosting and the most targeted organ for the hypertension processes in the human being. The complication entails various interactions that consist of the multiple organ systems and many mechanisms of autonomous or inter-reliant pathways. The factors that play a vital part in the pathogenesis of hypertension genetics, obesity, increase in the dietary slat uptake, the activation of the neurohormonal system, diabetes, and family history. More so, hypertension is grouped into four categories, namely, regular, mild, moderate, and server, where the treatment depends on the type where the pressure steadily falls when the data reading is taken.
For instance, among all the adults around the population of 78 million have been diagnosed with hypertension complications in the United States. Brown Hood is my college friend, who once introduces me to her grandmother, who had hypertension complications. Her grandmother has been having the hypertension health problem for the recording period six years from the commence of the difficulty. She was diagnosed after being diabetic for two years. The health problem has been disturbing her for a while. With better health choices and more management of other conditions though following the professional advice, she has well managed the situation.
Various goals are more appropriate and relevant to the health problem. Mostly the goal facilities the nurse to come up with plans that create the nursing business plans in the execution of various responsibilities to care for the patients. Thus, it makes them more focused on the career path and provides the proficient services that follow the articulated codes and ethics in practice (Hullmann, Robb, & Rand, 2016). Thus to enhance better handling of the health problem, I will be more specific. In this regard, I will ensure that I set the more proficient and competent nursing goal that will enhance better interpretation. The easy presentation will ensure that I provide better healthcare for the health problem associated with the patient. The deal will facilitate better delivery of the services, thus creating the nursing family relation among me, the patient, and the family’s members. More so, I will keep the most measurable goal that is effective and readily measured by the progress. The better continuous delivery in the associated health problem will give mu-conscious to celebrate the victory of individual achievement and maintain the motivation moral as the milestone will be tremendous. Moreover, I will make the goal that is more attainable in the health problem that I am dealing with, which does not limit my ability to handle. Thus I will ensure that I give myself more concentration time to break down the target goal, which will be more manageable in healthcare for the health problem that I am handling.
The health problem will be so relevant in my practice. The skill and the knowledge that I will gather from the medication will boost my career far and wide. Thus, I will gain more expertise that will steadily increase my analytical ability concerning health complications and guide me on the various ways to handle hypertension cases (Taylor, & Guerin, 2019). More so, I will be able to give multiple advisory opinions to the community on the habit of living that can make them eradicate and reduce the cases of the health problem. The advocacy will help me implement various activities that promote better health and prevent the alleviative illness that can occur as a result of under the control of the health problem. More so, health planning will boost my planning skills and focus on the outcome during my practice. The range of the outcome occurrence will equip me with better techniques to advance my methods to address the health problems associated with the patient. The move will be developed by the natural and faster diagnosis of the specific health problem.
