Program evaluation refers to a systematic collection of information about the activities involved, characteristics and the outcomes of any given program. Program evaluation is done to make a judgment about a program so that it can be improved for effectiveness and to inform about the program decisions. During the evaluation, some evaluation models are employed, and in our case, the logic model will be used to assess whether the program met its objectives. The logic model can be used for both developments of the program and evaluation since it captures the situation of the program, program inputs, program outputs, all the assumptions made, external factors likely to affect the success of the program and the overall outcomes. During the evaluation, the model assists in assessing whether the program achieved its set objectives so that changes can be made based on program decisions made. All community-based health programs are extensive and require an evaluation model that will bring out the actual outcome and guide on which decision to be made.
The health promotion theory that I will use in the proposed community project is the social cognitive theory. People are likely to make health decisions based on their individual experiences, the actions of those around them and the environmental factors. The theory is the best for community project since it offers an opportunity for social support to instill expectations to individual populations, increase self-efficacy and employ behavioral learning and other reinforcements to allow the individuals achieve behavior change. community-based health programs required social workers intervention and social cognitive theory is the best for instilling expectations to the community so that they can accept the initiated health program.
