Discussion of Geriatric Care Models
Various models of geriatric care are geared towards enhancing social and emotional support rather than medical support for the elderly. The models apply differently depending on the site of care. For instance, assisted living differs significantly from other approaches of caring for aging adults such as home care, rehabilitation facilities, and long-term care. At my current practicum site, assisted living is used to provide the seniors with enhanced daily living activities, hygiene, medication management, and nutritional support, among others (XXXX). It is, therefore, a holistic approach that focuses on more than a single domain of wellbeing.
The difference between assisted living sites and rehabilitation facilities is that the former admits individuals with general weaknesses to ensure sustained well-being. In contrast, the latter admit persons with past physical, medical, and other complications for recovery. On the other hand, home care seeks to provide the basic physiological and psychological support to the elderly within their residences to enhance their wellness and comfort as well as to prevent the complication of the already weakening body. Finally, long-term care facilities admit geriatric clients to provide necessary treatment and management of terminal and incurable complications (XXX). In general, all models of geriatric care focus on enhancing the general wellness of elderly adults.
The role of advanced nurses differs depending on the geriatric care model and site of care. Whereas advanced nurses in assisted living and home care geriatric facilities is to provide the essential care services largely entailing physical and psychological care, rehabilitation facilities, hospitals, and long-term care require more advanced skills for the management of the apparent patients’ medical complications. For the patient in an assisted living facility, care should be facilitated by gradually promoting patients’ capacity to perform basic self-care activities while providing them with the necessary psychological support (XXX). Such an approach enhances the easy realization of the goals of elderly care in assisted living facilities.
